Centralized Nurse Triage (RN or LPN)


As a member of our interdisciplinary team, CNT nurses provide a phone-based care coordination, triage, and clinical care. 

This is a full-time (40-hour/week) position. Monday-Friday hours


Responsible for providing direct clinical nursing services to patients by utilizing a team based model


  1. Triage patient telephone calls and walk-in patients as appropriate
  2. Provide nursing assessment, manage calls and provide care and advice to patients.
  3. Utilize clinical judgment and the nursing process
  4. Guided by medically approved protocols to determine the urgency of the patient’s problem and to direct the patient to the appropriate level of care
  5. Plan of care is developed in collaboration with the patient and included patient education and/or advice as appropriate and follow up as indicated
  6. Participates in Quality Improvement (QI), Quality Assurance (QA) and Continuous Quality Improvement Activities (CQI) as appropriate.
  7. Other duties as assigned


Education & Experience:

Active license in the State of Tennessee as a Registered Nurse; Licensed Practical Nurse with 5 years experience. Strong assessment skills; experience in phone triage; ability to work independently.

Demonstrated skills:

Demonstrate the ability to assess data reflective of the patient’s status and interpret the appropriate information to identify each patient needs to provide or direct the care needed.

Communication Skills:

Must possess the ability to communicate verbally and non-verbally with clients, coworkers and members of management. Excellent professional customer service skills required.

Computer Skills:

To perform this job successfully, an individual should have knowledge of Work Processing software, Electronic Medical Records software and Spreadsheet design software.
