PrEP NAVIGATOR

Location: (Knox Co./Knoxville)

Summary: 

A PrEP Navigator will work with behavioral and primary care providers, Community Health and Access Coordinators and Patient Assistance Representatives to provide intensive care management and coordination support to individuals who are at-risk of HIV/STI infection and who seek assistance accessing and remaining engaged in PrEP care.  The PrEP Navigator will have specialized knowledge about sexual health, PrEP, PEP, HIV, STI, harm reduction and the Ryan White program and will provide patient-centered, individualized support to individuals who are at-risk of HIV/STI infection and/or who are living with HIV.  The PrEP navigator will provide outreach, screening, education, navigation, engagement, and coordination services for at-risk populations and patients who receive PrEP prescriptions.  The PrEP Navigator will collaborate with patients receiving PrEP or treatment for HIV to address barriers to care access including but not limited to access, transportation, and resources and will collaborate with behavioral and primary care providers to engage HIV negative patients in preventative care. 

The PrEP Navigator will be expected to complete the PrEP Navigation Certification program and the I Know HIV programs within 90 days of start date.

Responsibilities: 

  • Provide advocacy, linkage, and referral to CHS and community resources.
  • Develop working relationships with appropriate community resources including those providing services to the target populations.
  • Promote health education and encourage patients to establish a healthcare home.
  • Educate and encourage patients to engage in preventative care and establish a patient-centered treatment plan.
  • Complete assessments to identify readiness for PrEP and collaborate with patients to develop plan to overcome barriers to readiness.
  • Assess benefits and insurance, and if needed, provide assistance to access payor programs.
  • Provide ongoing follow-up to support PrEP adherence.
  • Acquire and utilize Motivational Interviewing skills in the provision of services.
  • Identify barriers patients have in accessing health services including transportation and help patients develop plans to overcome barriers.
  • Help patients navigate the health care system. 
  • Maintain updated information about other local PrEP and PEP providers.
  • Collaborate with community organizations and businesses to participate in outreach events.
  • Enter data in NextGen and REDCap accurately and in a timely manner.
  • Complete reporting requirements as required/requested.
  • Maintain professional relationships with patients and with other staff and maintain appropriate interpersonal boundaries.
  • Refer patients to the appropriate staff for issues that fall outside the scope of PrEP navigation services.
  • Work effectively as a member of a multidisciplinary team using a unified care plan.
  • Document patient care including consent, assessment, intervention, and care coordination in a timely manner.
  • Respond in real-time to meet with patients with needs identified during other CHS appointments, develop plans to address, and coordinate care. 
  • Participate in supervision meetings and other meetings as designated by supervisor. 
  • Participate in quarterly conference calls, per grant-funding. 
  • Participate in Quality Improvement (QI), Quality Assurance (QA), and Continuous Quality Improvement Activities (CQI) as appropriate; and
  • Other duties as assigned.

QUALIFICATIONS

Education & Experience

  • Bachelor’s degree from a four-year university
  • 1-2 years of previous experience linking patients with various community resources preferred
  • Current or previous experience with PrEP/HIV services preferred
  • Valid driver’s license and a clean driving history

Communication Skills:

Must possess the ability to speak to individuals on 1:1 basis or in a group

Computer Skills

To perform this job successfully, an individual should have knowledge of Word Processing software.
