Triage Nurse




Supports the Cherokee Health Systems mission by facilitating patient access to integrated primary and behavioral health services by utilizing a team-based mode. 

RESPONSIBILITIES include the following:

The following duties are normal for this job. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.

  • Utilizes sound clinical judgment, the nursing process and telephonic nurse triage software to assist patients to navigate the CHS healthcare system.
  • Follows medically approved triage protocols (Schmitt-Thompson) to assign patients to the appropriate level of care: (Emergent, Urgent, Non-Urgent, or Home Care)
  • Uses the EHR and triage software to provide patient education material appropriate to the patient’s medical and nursing diagnoses.
  • Uses telephone queue to manage incoming calls efficiently and promptly. Monitors own performance by keeping a weekly call log.
  • Utilizes the EHR (Nextgen) to access the patient’s medical history, orders, guidelines, dashboard, and plan of care, to schedule patient for services, and to communicate with other staff who are participating in the patient’s care.
  • Uses excellent communication and customer service skills to provide a high-quality patient experience. Assesses patient’s health care literacy and responds in appropriate non-clinical language to facilitate patient’s understanding.
  • Facilitates care coordination by identifying new or existing patients with high acuity. Makes appropriate referrals within and outside of the department to ensure care coordination
  • Participates in quality improvement and training activities as assigned.
  • Follows all policies and procedures established for the Central Nurse Triage Department.
  • Other duties as assigned.

 • Always maintain patient confidentiality.
 • Report to work on time and as scheduled. Consistent and regular attendance is an essential function of this position.
 • Wear identification while on duty.
 • Maintain regulatory requirements, including all state, federal and local regulations.  
 • Always represent the organization in a positive and professional manner.
 • Comply with all organizational policies and standards regarding ethical business practices.  
 • Participate in performance improvement and continuous quality improvement activities.
 • Attend regular staff meetings and in-services as needed.  


Education & Experience

TN licensed as a Registered Nurse or Licensed Practical Nurse with minimum three years of practical experience preferred.

Demonstrate a high level of fluency in Spanish and English, both written and spoken.

Certificates and Licenses:

Licensed: Registered Nurse or Licensed Practical Nurse with State of Tennessee. Maintain professional license.

Certification in CPR and OSHA training.

Current, unexpired TN Driver’s License with clean driving history (if applicable- some jobs may not require working at multiple locations). 

Computer Skills

To perform this job successfully, an individual should have working knowledge of Microsoft Office and Electronic Medical Records software.

Knowledge, Skills, and Abilities:


  1. Knowledge of medical terminology, anatomy, physiology, and pathophysiology. Familiarity with health care systems, regulations, policies, and functions. Understanding of documentation standards.
  2. Knowledge of equipment, supplies, and materials needed for medical treatment. Understanding of basic laboratory procedures including preparation and screening. 
  3. Knowledge of infectious disease management and control and safety standards.


  1. Skill in reading and following written and oral medical orders.
  2. Skill in initiating appropriate emergency procedures.
  3. Skill in handling a number of tasks simultaneously.
  4. Skill in performing blood draws.


  1. Ability to use manual dexterity to perform medical treatments.
  2. Ability to establish and maintain effective working relationships with coworkers and diverse patient populations.
  3. Ability to perform mathematical calculations for drug dosages.
